SHIP SMP Guidance 2008

Recognizing that SMP grantees who are also SHIP programs have unique reporting concerns, CMS and AoA, with assistance from the SMP Resource Center, produced a guide to reporting in SMART FACTS (SMP database) and NPR (SHIP database). The final two pages of the guide contain a table that will aid data entry staff in understanding what data goes into SMART FACTS, what data goes into NPR and what data goes into both systems.  This document is also helpful for understanding the unique missions of these two separately funded programs.

SMART FACTS Reports: Analyzing and Compiling Data

  • Targeted Populations Report
    This new report in SMART FACTS allows you to calculate the total number of people served when conducting group educational sessions and one-on-one counseling sessions targeted toward hard-to-reach populations. The reports are broken down according to each type of population. 
  • Dissemination Activities Report
    This new report in SMART FACTS totals the number of items disseminated, broken down by each type of item (brochures, personal health care journals, etc.).
