Integration Grantees:  Homebound and Long Term Care Issues

 

Organization Name: Diocese of Providence

Target:  Homebound Seniors

Target Issues:   Our goal is to reach homebound elders of three cities within the Diocese of Providence by using the Friendly Visitors to more fully integrate beneficiary education and awareness of Medicare and Medicaid errors, fraud and abuse.

Products Produced: Recruiting Brochure (Spanish and English) and “How to establish a friendly visitor program” Guide

Partners:  Rhode Island Alzheimer’s Association, Rhode Island Dept of Elderly Affairs, Capital Region RSVP, Quality Partners of Rhode Island

 

Organization Name: Multi-Service Center

Target: Hispanic/Latino, Native American, Long-Term Care Facility Residents

Target Issue: Long-term care and health care rights, fraud and abuse prevention, coordination with law enforcement and development of local fraud and abuse prevention councils

Partners:  Resident councils of Washington State and Alzheimer’s Association of Western Washington

 

Organization Name: Maine Department of Health and Human Services

Target: Rural, Low Literacy, Caregivers, Homebound, French speaking staff, residents and caregivers in long term care facilities

Target Issue:     DME Fraud and Part D Fraud

Products Produced: Two brochures (DME and Part D), Two TV ads (DME and Part D)

Partners:  Maine LTC Ombudsman Program, Maine AAA, Legal Services for the Elderly, Maine AG Office

 

Organization Name: AgeOptions

Target: Rural, Low Literacy, Homebound

Target Issue:  Creating SMP materials for low health literacy older adults and testing the materials in focus groups.  Materials will be disseminated through partners.

Products Produced: Prescription Bag Insert, SMP Brochure, Health Care Fraud Intake Form, Postcard, SMP poster

Partners: Meals on Wheels, Local grocery store chain, US Postal Inspection Service- Fraud Division
