Improper billing occurs when doctors, hospitals, or other providers charge beneficiaries with both Medicaid and Medicare for copays, coinsurance, or deductibles. Patients who have both Medicaid and Medicare (including Medicare Advantage) should never be charged for services covered under Medicaid or Medicare.
